ფარმაკოლოგიური თვისებები:

ბისოპროლოლი არის ბეტა1-ადრენორეცეპტორის მაღალსელექტიური ანტაგონისტი, ბეტა-2-რეცეპტორებთან დაბალი მსგავსებით. არ გააჩნია სიმპათომიმეტიკური და მემბარანომასტაბილიზებელი თვისებები. გააჩნია ჰიპოტენზიური და ანტიანგინალური მოქმედება, ამცირებს მიოკარდიუმის მოთხოვნას ჟანგბადზე.

ჰიპოტენზიური მოქმედება დაკავშირებულია პერიფერიული სისხლძარღვების სტიმულირებასთან, რენინ-ანგიოტენზინის სისტემის აქტივობის შემცირებასთან (მნიშვნელოვანია რენინის ჰიპერსეკრეციის მქონე პაციენტებისათვის) და სისხლის წუთობრივი რაოდენობის შემცირებასთან. 

ანტიანგინალური მოქმედება განპირობებულია ჟანგბადზე მიოკარდიუმის მოთხივნილების შემცირებით (გულის შეკუმშვათა სიხშირის და კუმშვადობის შემცირება), დიასტოლის გახანგრძლივებით და მიოკარდიუმის პერფუზიის გაუმჯობესებით.

არითმიის საწინააღმდეგო ეფექტი დაკავშირებულია არითმოგენული ეფექტების შემცირებით (ტაქიკარდია, სიმპატური ნერვული სისტემის მომატებული აქტივობა), AV-გამტარებლობის შენელებით.

 

ფარმაკოკინეტიკა:

ბისოპროლოლი შეიწოვება თითქმის სრულად კუჭ-ნაწლავის ტრაქტიდან. ახასიათებს მაღალი ბიოშეღწევადობა - 90%. ბისოპროლოლის პლაზმურ ცილებთან შეკავშირება შეადგენს დაახლოებით 30%-ს. განაწილების მოცულობა არის 3.5 ლ/კგ. საერთო კლირენსი შეადგენს დაახლოებით 15 ლ/სთ.

პლაზმაში ნახევარგამოყოფის პერიოდი (10-12 საათი) უზრუნველყოფს 24-საათიან ეფექტუობას  დღეში ერთჯერადი დოზირებისას.

ბისოპროლოლი მეტაბოლიზდება ღვიძლით, გამოიყოფა თირკმლებით.  50 % გამოიყოფა უცვლლი სახით, ხოლო 50%- არააქტიური მეტაბოლიტების სახით.

• ქრონიკული გულის უკმარისობა აგფ ინჰიბიტორებთან, დიურეტიკებთან და არჩევითად, საგულე გლიკოზიდებთან კომბინაციაში.

 

დოზირება და მიღების წესი:

ჩვეულებრივი საწყისი დოზა შეადგენს 5 მგ დღეში ერთხელ. 

ზოგიერთ პაციენტში 2.5 მგ შეიძლება იყოს საკმარისი საწყისი დოზა. 5 მგ მიიღება დღეში ერთხელ დილით, უზმოზე ან საუზმის დროს. აუცილებლობისას, დოზა შეიძლება გაიზარდოს 10 მგ-მდე დღე-ღამეში. იშვიათად შეიძლება საჭირო გახდეს დოზის მომატება დღეში 20 მგ-მდე.

დოზა ყოველთვის შეირჩევა ინდივიდუალურად,  გულის შეკუმშვების სიხშირისა და თერაპიული ეფექტის გათვალისწინებით. აუცილებელი არ არის დოზის კორექცია ღვიძლის ან თირკმლის ფუნქციის მსუბუქი ან საშუალო სიმძიმის დარღვევების მქონე პაციენტებში. 

თირკმლის ფუნქციის მძიმე დარღვევების მქონე პაციენტებში (კრეატინინის კლირენსი <20 მლ/წთ) და ღვიძლის ფუნქციის მძიმე დარღვევის მქონე პაციენტებში მაქსიმალური დღიური დოზა შეადგენს 10 მგ-ს.  ასევე საჭიროა ბისოპროლოლის  დოზის კორექცია ხანდაზმულ პაციენტებში.

ცენტრალური ნერვული სისტემის მხრივ: თავბრუსხვევა, ხასიათის ცვალებადობა, ვერტიგო, სინკოპე, თავის ტკივილი, პარესთეზია, ჰიპესთეზია, ჰიპერესთეზია, ძილიანობა, უძილობა, შფოთვა/მოუსვენრობა, კონცენტრაციის/მეხსიერების დაქვეითება, დეპრესია, უჩვეულო სიზმრები.

გულ-სისხლძარღვთა სისტემის მხრივ: ბრადიკარდია, პალპიტაციები და რითმის სხვა სახის დარღვევები, კიდურების გაციება, კოჭლობა, ჰიპოტენზია, ორთოსტატული ჰიპოტენზია, ტკივილი გულმკერდის არეში, გულის უკმარისობა, დისპნოე ფიზიკური დატვირთვისას.

ერთეულ შემთხვევებში აღინიშნებოდა:

ცენტრალური ნერვული სისტემის მხრივ: შექცევადი დეპრესია (კატატონიამდე), ჰალუცინაციები, მწვავე შექცევადი დეზორიენტაციის სინდრომი, ემოციური ლაბილურობა, მგრძნობელობის მსუბუქი დარღვევა.

ალერგიული რექციები: ცხელება, ყელის ტკივილი, ლარინგოსპაზმი, სუნთქვის გაძნელება.

ჰემატოლოგია: აგრანულოციტოზი, თრომბოციტოპენია, თრომბოციტოპენული პურპურა.

კუჭ-ნაწლავის ტრაქტის მხრივ: მეზენტერიული არტერიული თრომბოზი, იშემიური კოლიტი.

 

უკუჩვენებები:

გულის მწვავე უკმარისობა, გულის ქრონიკული უკმარისობა დეკომპენსაციის სტადიაში, კარდიოგენული შოკი, კოლაფსი, II და III ხარისხის AV- ბლოკადა, სინო-ატრიალური ბლოკადა, ბრადიკარდია (<50 ნაკლები), პრინცმეტალის სტენოკარდია, გამოხატული ჰიპოტენზია (სისტოლური <90 ნაკლები), ბრონქული ასთმის მძიმე ფორმა, პერიფერიული სისხლის მიმოქცევის მძიმე დარღვევები, რეინოს დაავადება, ფეოქრომოციტომა, მეტაბოლური აციდოზი, მაო-ს ინჰიბიტორებთან ერთდროული გამოყენება, 18 წლამდე ასაკი, ორსულობა და ლაქტაცია, მომატებული მგრძნობელობა ბეტა-ბლოკერების მიმართ. 

ჩვეულებრივი დოზა უნდა შემცირდეს ხანდაზმულებში ან თირკმლის დისფუნქციის მქონე პაციენტებში.

მეტაბოლური აციდოზისა და სინუსური ბრადიკარდიის მქონე პაციენტები.

უსაფრთხოება და ეფექტიანობა დადგენილი არ არის. 

 

სიფრთხილის ზომები:

ოპერაციის წინ ბისოპროლოლის მოხსნისას ბოლო დოზასა და ანესთეზიას შორის უნდა გავიდეს მინიმუმ 48 საათი. თუ ბისოპროლოლით მკურნალობა  გრძელდება ოპერაციის პერიოდში, საანესთეზიო საშუალებები (ციკლოპროპანი და ტრიქლოროეთილენი) გამოიყენება სიფრთხილით. ვაგუსური მდგომარეობის კორექცია უნდა მოხდეს ატროპინით (1-2 მგ ი.ვ.). 

პერიოპერაციულ პერიოდში მიზანშეწონილი არ არის დოზირების შემცირება, ვინაიდან არსებობს სტენოკარდიის ან ჰიპერტენზიის გამწვავების რისკი.

ოპერაციის დროს ან მის შემდეგ შეიძლება შეიცვალოს პაციენტის  პასუხი ჰიპოვოლემიაზე ან სისხლის დაკარგვაზე (ტაქიკარდიის მხრივ). ამ შემთხვევაში აუცილებელია განსაკუთრებული სიფრთხილის გამოჩენა.

გულის იშემიური დაავადების მქონე პაციენტებში, არ შეიძლება მკურნალობის უეცარი შეწყვეტა.

სიფრთხილის ინიშნება ბისოპროლოლი 1 კლასის ანტიარითმულ საშუალებებთან  ერთად, როგორიცაა დისოპირამიდი და კალციუმის ანტაგონისტებთან.

სიფრთხილით გამოიყენება ვერაპამილთან კომბინაციაში ვენტრიკულური ფუნქციის დარღვევის მქონე პაციენტებში. ამგვარი კომბინაცია არ გამოიყენება გამტარობის დარღვევების მქონე პაციენტებში. არც ერთი პრეპარატი არ უნდა იქნას შეყვანილი ინტრავენურად მეორე პრეპარატის შეწყვეტიდან 48 საათის განმავლობაში.

ბისოპროლოლით თერაპიისას რეკომენდებული არ არის კალციუმის ანტაგონისტებისა და ანტიარითმიული საშუალებების ინტრავენური შეყვანა.

აუცილებელია სიფრთხილის გამოჩენა პაცინეტის კლონიდინით მკურნალობიდან გადმოყვანისას. კლონიდინის მოხსნამ შეიძლება გამოიწვიოს კატექოლამინების დიდი რაოდენობით გამოთავისუფლება, რამაც შეიძლება გამოიწვიოს ჰიპერტონული კრიზი. თუ ბეტა-ბლოკატორი და კლონიდინი გამოიყენება ერთდროულად, ბეტა-ბლოკატორის მოხსნიდან რამდენიმე დღის შემდეგ უნდა შეწყდეს კლონიდინის მიღება, ვინაიდან შეიძლება განვითარდეს ჰიპერტენზია.

ბისოპროლოლის დოზა უნდა დაკორექტირდეს თირკმლის ფუნქციის მძიმე დარღვევის მქონე პაციენტებში.

ორსულობა:

მშობიარებამდე ცოტა ხნით ადრე ან მშობიარობის დროს ბისოპროლოლის მიღება ორსულ ქალებში იწვევს ახალშობილის ჰიპოტონიას ან ჰიპოგლიკემიას.

 

სპეციალური სიფრთხილის ზომები:

მკურნალობის უეცარმა შეწყვეტამ შეიძლება გამოიწვიოს სტენოკარდიის შეტევათა გახშირება გულის იშემიური დაავადების მქონე პაციენტებში. თერაპიის შეწყვეტა უნდა მოხდეს თანდათანობით და პაციენტმა უნდა შეიზღუდოს ფიზიკური აქტივობა პრეპარატის მოხსნის პერიოდში. 

ბრონქოკონსტრიქცია შეიძლება განვითარდეს ასთმის, ბრონქიტისა და სხვა ქრონიკული პულმონური დაავადების მქონე პაციენტებში. ვინაიდან ბისოპროლოლი არის ბეტა 1-ადრენორეცეპტორის მაღალსელექციური ბლოკატორი, მისი გამოყენება სიფრთხილით შეიძლება ქრონიკული ობსტუქციული დაავადების მქონე პაციენტებში. ბრონქოსპაზმის მოხსნა შესაძლებელია ფართოდ გამოყენებადი ბრონქოდილატატორებით. 

ბისოპროლოლმა შეიძლება შენიღბოს ჰიპერთირეოიდიზმის სიმპტომები.

სიფრთხილით გამოიყენება ჰიპოგლიკემიის მქონე პაციენტებში. 

 

ურთიერთქმედება სხვა სამკურნალო საშუალებებთან

შეიძლება სახიფათო იყოს ბისოპროლოლის ერთდროული მიღება შემდეგ პრეპარატებთან:

ჰიპოგლიკემიური საშუალებები, ფენოთიაზინები და სხვადასხვა ანტიარითმიული საშუალებები. N.B. – ამგვარ ურთიერთქმედებას შეიძლება ჰქონდეს სიცოცხლისათვის საშიში შედეგები. 

შეიძლება გააძლიეროს ჰიპოგლიკემიური საშუალებების ეფექტები შაქრიანი დიაბეტის მქონე პაციენტებში, აგრეთვე მიოკარდიუმის დეპრესანტების – ლიგნოკაინის, პროკაინამიდისა და ქინიდინის ეფექტები. 

ეფექტების ანტაგონიზმი შეიძლება მოხდეს ბეტა-ადრენორეცეპტორების სტიმულატორების მიერ (მაგ. იზოპრენალინი). ადრენომიმეტიკები ამცირებენ ბისოპოლოლის მოქმედებას. ადრენობლოკატორებს (გუანეთიდინი და რეზერპინი) შეუძლია  ეფექტის გაძლიერება.

ოპერაციის ჩატარებამდე უნდა ეცნობოს ანესთეზიოლოგს ბისოპროლოლით მკურნალობის შესახებ.

ბისოპროლოლის ნახევარგამოყოფის პერიოდი შეიძლება შემცირდეს რიფამპიცინის იმავდროული მიღებით. დოზის მომატება აუცილებელი არ არის. ციმეტიდინი არ ახდენს მნიშვნელოვან გავლენას ბისოპროლოლის ფარმაკოკინეტიკაზე.  

 

ჭარბი დოზირება:

ჭარბი დოზირების სიმპტომებია: ბრადიკარდია და მძიმე ჰიპოტენზია, შესაძლებელია ბრონქოსპაზმის და გულის უკმარისობის განვითარება.

მძიმე ჰიპოტენზიასთან ასოცირებული ბრადიკარდიის მკურნალობა უნდა მოხდეს ატროპინის ინტრავენური შეყვანით (1-2 მგ). აუცილებლობისას შეიძლება 25 მგ იზოპრენალინის შეყვანა ნელი ინტრავენური ინფუზიის სახით. ბრნქოსპაზმის მოხსნა უნდა მოხდეს ინტრავნური ამინოფილინით, ხოლო გულის უკმარისობის მკურნალობა – საგულე გლიკოზიდებით და დიურეტიკებით.

 

შეფუთვა:

შენახვის პირობები:

ინახება 25oC-მდე ორიგინალ შეფუთვაში, ბავშვებისათვის მიუწვდომელ ადგილას.
