ფარმაკოლოგიური თვისებები

ცეფიქსიმი არის პერორალური ბეტა-ლაქტამაზას მიმართ სტაბილური მესამე თაობის ცეფალოსპორინი ფართო სპექტრითა და ბაქტერიციდული მოქმედებით მრავალი გრამდადებითი და გრამუარყოფითი მიკროორგანიზმის მიმართ.

ცეფიქსიმი ეფექტურია მრავალი სახეობის მიკროორგანიზმებით გამოწვეული ინფექციების სამკურნალოდ, როგორიცაა: სტრეპტოკოკი, Neisseria gonorrhoeae, Branhamella catarrhalis, Escherichia coli, Klebsiella, Proteus da Haemophilus influenzae. მისი მოქმედების სპექტრი და MIC მაჩვენებლები მესამე თაობის სხვა ცეფალოსპორინების მსგავსია. 

ცეფიქსიმის ბაქტერიციდულ მოქმედებას განაპირობებს უჯრედოვანი კედლის სინთეზის დათრგუნვა. ცეფიქსიმი სტაბილურია ბეტა-ლაქტამაზას ფერმენტების არსებობისას და შედეგად, ბეტა-ლაქტამების არსებობით გამოწვეული პენიცილინებისა და ზოგიერთი ცეფალოსპორინის მიმართ რეზისტენტული მრავალი შტამი, შეიძლება მგრძნობიარე იყოს ცეფიქსიმის მიმართ.

ცეფიქსიმი აქტიურია ინ ვიტრო შემდეგი ორგანიზმების წინააღმდეგ:

გრამდადებითი ორგანიზმები: Streptococcus pyogenes, Streptococcus pneumonia, Streptococcus agalactiae.

გრამუარყოფითი ორგანიზმები: Haemophilus influenzae, Haemophilus parainfluenzae, Branhamella catarrhalis, Neisseria gonorrhoeae, Escherichia coli, Proteus mirabilis, Proteus vulgaris, სხვა ინდოლ-დადებითი Proteus-ის სახეობები, Klebsiella pneumoniae, Klebsiella oxytoca, Enterobacteriaceae, Pasteurella multocida, Providencia-ს სახეობები, Salmonella-ს სახეობები, Shigella-ს სახეობები, Citobacter amalonaticus, Citobacter diversus, Serratia marcescenes.

 

ფარმაკოკინეტიკა

პერორალური მიღების შემდეგ ცეფიქსიმის აბსორბციის სიჩქარეზე გავლენას არ ახდენს საკვები და ცეფიქსიმი იოლად აბსორბირდება. ცეფიქსიმის ფარმაკოკინეტიკაზე გავლენას არ ახდენს ასაკი.

200-2000 მგ-ს ფარგლებში დოზირებისას ადგილი აქვს უმაღლესი შრატისმიერი კონცენტრაციისა და კონცენტრაციის დროის დახრის უბნების სწორხაზოვან ზრდას. 

ერთჯერადი 200 ან 400 მგ დოზის პერორალური მიღებიდან 3-4 საათის შემდეგ, უმაღლესი შრატისმიერი კონცენტრაცია შეადგენდა 2-4 მკგ/მლ-ს და 3-5 მკგ/მლ-ს შესაბამისად. არ დასტურდება მრავალჯერადი დოზების მიღების შემდეგ შრატსა და შარდში ცეფიქსიმის დაგროვების ფაქტი. აბსორბირებული დოზის 50% უცვლელი სახით გამოიდევნება შარდთან ერთად 24 საათის განმავლობაში.

დოზის 10% ექსკრეტირდება ნაღველში. ცეფიქსიმის 65% უკავშირდება შრატისმიერ პროტეინებს, ხოლო შრატისმიერი  ახევარგამოყოფის პერიოდი ჯანმრთელ მოზრდილებში შეადგენს 3-4 საათს.

თირკმლის ზომიერი უკმარისობისას (კრეატინინის კლირენსი 20-40 მლ/წთ) და ზოგიერთი სახის თირკმლის უკმარისობისას (კრეატინინის კლირენსი 5-20 მლ/წთ) შრატისმიერი ნახევარგამოყოფის პერიოდი ხანგრძლივდება და შეადგენს 6.4 და 11.5 საათს შესაბამისად.

ცეფიქსიმის მხოლოდ ძალიან მცირე რაოდენობა გამოიდევნება ჰემოდიალიზით ან პერიტონეალური დიალიზით. ცეფიქსიმი არ მეტაბოლიზდება ინ ვივო.

ჩვენებები

ცეფიქსიმი ნაჩვენებია მგრძნობიარე მიკროორგანიზმებით გამოწვეული შემდეგი ინფექციების სამკურნალოდ:

- რესპირატორული ტრაქტის ინფექციები: ტონზილიტი, ფარინგიტი, შუა ყურის ანთება, სინუსიტი, მწვავე პნევმონია,

- მწვავე და ქრონიკული ბრონქიტი.

- საშარდე ტრაქტის ინფექციები: მწვავე ცისტიტი, ურეთრიტი, პიელონეფრიტი.

- ცეფიქსიმს გააჩნია ბეტა-ლაქტამაზას მიმართ სტაბილურობის მაღალი ხარისხი. 

 

დოზირების რეჟიმი

ცეფილონი არის პერორალური ანტიბიოტიკი და ცეფიქსიმის აბსორბციის სიჩქარეზე საკვები გავლენას არ ახდენს. მოზრდილები და ბავშვები 12 წლის ზემოთ: ჩვეულებრივი დოზა შეადგენს დღეში 1 ტაბლეტს,(400 მგ, )რომელიც მიიღება ერთჯერადი დოზის სახით ან 200 მგ ყოველ 12 საათში.

საშარდე ტრაქტის გაურთულებელი ინფექციების სამკურნალოდ გამოიყენება 200 მგ დოზა. 

დოზირება ხანდაზმულ პაციენტებში: გამოიყენება მოზრდილებისათვის რეკომენდებული დოზა. თირკმლის უკმარისობის მქონე პაციენტებში საჭიროა დოზის კორექცია. 

- დოზირება ბავშვებში: 2 წლამდე ასაკის ბავშვებში რეკომენდებული დოზა არის 8 მგ/კგ/დღეში, რომელიც მიიღება ერთჯერადი ყოველდღიური დოზის სახით და ორ დოზად დაყოფილი, ყოველ 12 საათში. 

- 50 კგ-ზე მეტი წონის ბავშვებში ან ბავშვებში 12 წლიდან, რეკომენდებულია მოზრდილებში გამოსაყენებელი დოზა. 6 თვემდე ასაკის ბავშვებში ცეფიქსიმის გამოყენების უსაფრთხოება და ეფექტურობა არ არის დადგენილი.

გვერდითი მოვლენები

400 მგ ცეფილონის შემოგარსული ტაბლეტები ზოგადად ხასიათდება კარგი ამტანობით. არასასურველი ეფექტების უმრავლესობა საშუალო სიმძიმისაა.

ეფექტები კუჭ-ნაწლავის ტრაქტის მხრივ: ყველაზე ხშირად აღინიშნება დიარეა, რომელიც საშუალო ან ზომიერი სიმძიმისაა და იშვიათად საჭიროებს მკურნალობის შეწყვეტას.

ნაკლებად ხშირად აღინიშნება გულისრევა, ტკივილი მუცლის არეში, დისპეფსია, პირღებინება და მუცლის შებერილობა.

ფსევდომემბრანოზული კოლიტი იშვიათად აღინიშნება.

ეფექტები ცენტრალური ნერვული სისტემის მხრივ: თავის ტკივილი, თავბრუსხვევა.

ჰიპერმგრძნობელობითი რეაქციები: გამონაყარი კანზე, ქავილი, ჭინჭრის გამონაყარი, არტიკულარული ტკივილი და ტემპერატურა.

თერაპიის შეწყვეტისას აღნიშნული ეფექტები ქრება. 

ჰემატოლოგიური ეფექტები და ლაბორატორიული ანალიზების მაჩვენებლების ცვლილება: თრომბოციტოპენია, ლეიკოპენია, ეოზინოფილია. აღნიშნული რეაქციები იშვიათია და შექცევადი ხასიათის. აღინიშნება დროებითი ცვლილებები ღვიძლისა და თირკმლის ფუნქციის ანალიზებში.

მოულოდნელი გვერდითი ეფექტის განვითარებისას აუცილებელია ექიმთან კონსულტაცია.

უკუჩვენებები

ცეფიქსიმი უკუნაჩვენებია ანტიბიოტიკების ცეფალოსპორინების ჯგუფის  მიმართ დადგენილი ალერგიისას.

ორსულობა და ლაქტაცია

B კატეგორიის პრეპარატი. ცხოველებში ჩატარებული გამოკვლევებისას ცეფიქსიმის მაღალი დოზის გამოყენებისას არ აღინიშნა ტერატოგენული ეფექტები. არ არსებობს ადექვატური და კონტროლირებული გამოკვლევები და ამიტომ, ორსულობისა და ლაქტაციის პერიოდში ცეფიქსიმი გამოიყენება მხოლოდ იმ შემთხვევაში, თუ ის მკაფიოდ ნაჩვენებია.

განსაკუთრებული მითითებები

ცეფიქსიმი სიფრთხილით გამოიყენება სხვა პრეპარატის მიმართ ადრე გამოვლენილი ჰიპერმგრძნობელობითი რეაქციების შემთხვევაში. 

სიფრთხილის გამოჩენა არის საჭირო პენიცილინ-მგრძნობიარე პაციენტებში ცეფალოსპორინ-პენიცილინის ჯვარედინი მგრძნობელობის გამო. 

ალერგიული რეაქციების გამოვლენისას პრეპარატის მიღება უნდა შეწყდეს და დაიწყოს სათანადო თერაპია.

400 მგ ცეფილონის შემოგარსული ტაბლეტის დოზის კორექცია უნდა მოხდეს თირკმელების მწვავე უკმარისობის მქონე პაციენტებში. 

არსებობს რეზისტენტული ორგანიზმების წარმოქმნის ალბათობა, რის შედეგადაც შეიძლება აღინიშნოს მათი ჭარბი ზრდა, განსაკუთრებით ხანგრძლივი მკურნალობისას. ფართო სპექტრის ანტიბიოტიკებით, მათ შორის ცეფიქსიმით მკურნალობისას აღინიშნება ნაწლავური ფლორის ცვლილება და კლოსტრიდიას ჭარბი ზრდა. გამოკვლევებმა აჩვენა, რომ კლოსტრიდიის მიერ წარმოქმნილი ტოქსინი არის ანტიბიოტიკთან ასოცირებული დიარეის, მათ შორის ფსევდომემბრანოზული კოლიტის უპირველესი მიზეზი. 

მწვავე დიარეის დროს მკურნალობა უნდა შეწყდეს. 


ჭარბი დოზირება

ინფორმაცია ცეფიქსიმით ზედოზირების შესახებ არასაკმარისია.

არასასურველი რეაქციები, რომლებიც ვლინდება ჯანმრთელ პაციენტებში 2 გ-მდე ცეფიქსიმის ერთჯერადი მიღებისას, არ განსხვავდება იმ პაციენტებში გამოვლენილი რეაქციებისგან, რომლებიც მკურნალობას გადიან სტანდარტული დოზებით.

ნაჩვენებია კუჭის გამორეცხვა. სპეციფიკური ანტიდოტი არ არსებობს. ცეფიქსიმი მნიშვნელოვანი რაოდენობით არ გამოიდევნება სისხლის მიმოქცევიდან არც ჰემოდიალიზით და არც პერიტონული დიალიზით.

 

შენახვის პირობები და ვადა 

პრეპარატი ინახება არა უმეტეს 30ºC ტემპერატურაზე, ბავშვებისათვის მიუწვდომელ ადგილას.

ვარგისიანობის ვადა - 2 წელი.

პრეპარატი გაიცემა ექიმის რეცეპტით.
