ფარმაკოლოგიური თვისებები:

ესციტალოპრამი- ციტალოპრამისS-ენანთიომერი, სეროტონინის(5HT) უკუმიტაცების მძლავრი სელექტიური ინჰიბიტორია.  მისი ანტიდეპრესიული მექნიზმი დაკავშირებულია ცენტრალურ ნერვულ სისტემაში სეროტონინერგული აქტივობის პოტენცირებასთან:

ესციტალოპამი უკავშირდება სეროტონინის ტრანსპორტერის ( SERT) როგორც პირველად (ორთოსტერულ) ცენტრს,  ასევე მის ალოსტერულ ცენტრს. სეროტონინის ტრანსპორტერის ალოსტერული მოდულაცია აძლიერებს ესციტალოპრამის შეკავშირებას მის ორთოსტერულ ცენტრთან, რასც განაპირობებს სეროტონინის უკუმიტაცების თითქმის  სრულ ინჰიბირებას. [1]

სეროტონინის უკუმიტაცებისი ნჰიბირება იწვევს სინაფსურ ნაპრალში სეროტონინის კონცენტრაციის ზრდას, პოსტსინაფსურ რეცეპტორულ უბნებზე მისი მოქმედების გაძლიერებასა და პროლონგირებას. 

ესციტალოპრამი პრაქტიკულად არ ურთიერთქმედებს სეროტონინის 5-HT1A, 5-HT2-დადოფამინის D1დაD2, ადრენერგულα1-, α2-, დაβ, ჰისტამინურ Н1, მუსკარინულ, ბენზოდიაზეპინურ და ოპიოიდურ რეცეპტორებთან..

ესციტალოპრამი ასევე არ უკავშირდება ან თითქმის არ უკავშირდება Na+, K+, CI+ და Ca++ არხებს.

ესციტალოპამი ციტალოპრამის R-ენანთიომერთან შედარებით 100-ჯერ უფროეფექტურია, 5-HT უკუმიტაცების და 5-HT ნეირონულიაგზნებადობის დონის ინჰიბირების მიხედვით.

  

ფარმაკოკინეტიკა

ესცატილოპრამის ორალური ბიოშეღწევადობა დაახლოებით 100%-ია და მის აბსორბციაზე საკვები არ ახდენს გავლენას. კონცენტრაციის პიკი პლაზმაში მიიღწევა 4 საათში. ისევე როგორც რაცემიული ციტალოპრამის შემთხვევაში, ესციტალოპრამის ძირითადი მეტაბოლიტების ცილებთან კავშირის მაჩვენებელი 70%-ზე ნაკლებია. ესციტალოპრამი მეტაბოლიზდება ღვიძლში დემეთილციტალოპრამად და დიდემეთილციტალოპრამად. ორივე მეტაბოლიტი ფარმაკოლოგიურად აქტიურია. შეუცვლელი ესციტალოპრამი არის დომინანტური სტრუქტურა პლაზმაში. მას ახასიათებს ხაზოვანი კინეტიკა. პლაზმაში სტაბილური კონცენტრაცია მიიღწევა დაახლოებით ერთ კვირაში. პრეპარატის ნახევარგამოყოფის პერიოდი შეადგენს 30 საათს და კლირენსი არის დაახლოებით 0.6ლ/წთ. გამოყოფა უმთავრესად ხორციელდება შარდით და განავლით. მიღებული დოზის უმეტესი ნაწილი გამოიყოფა შარდში მისი მეტაბოლიტების სახით.

მიღების წესები და დოზები:

პრეპარატის მიღება ხდება დღეში ერთხელ, მისი მიღება შეიძლება დღის ნებისმიერ დროს საკვების მიღებისგან დამოუკიდებლად.

არ არის დადგენილი მისი უსაფრთხოება 20 მგ-ზე მაღალი დღიური დოზის მიღების შემთხვევაში.

დიდი დეპრესიული აშლილობა: ჩვეულებრივ იღებენ 10 მგ დღეში ერთხელ, დღიური დოზა შეიძლება გაიზარდოს 20 მგ-მდე პაციენტის ინდივიდუალური საჭიროებისა და დეპრესიის სიმძიმის შესაბამისად

ანტიდეპრესიული ეფექტი ჩვეულებრივ 2-4 კვირაში მიიღწევა. ანტიდეპრესანტებით მკურნალობა სიმპტომურია, ამიტომ პრეპარატი რეციდივების საპროფილაქტიკოდ 6 ან მეტი თვის განმავლობაში უნდა იქნას გამოყენებული.

შფოთვითი აშლილობა აგორაფობიით და აგორაფობიის გარეშე:  ესციტალოპრამით მკურნალობა იწყება 5 მგ-ით დღეში პირველი კვირის განმავლობაში და როგორც წესი იზრდება 10 მგ -მდე დღეში. მოგვიანებით დღიური დოზა შეიძლება გაიზარდოს 20 მგ-მდე დღეში პაციენტის ინდივიდუალური საჭიროებისა შესაბამისად.

მაქსიმალური ეფექტი მიიღწევა 3 თვეში. მკურნალობა გრძელდება რამდენიმე თვე.

სოციალური შფოთვითი აშლილობა (სოციალური ფობია): ჩვეულებრივ ინიშნება 10 მგ დღეში. სიმპტომთა შესამსუბუქებლად საჭიროა 2-4 კვირიანი მკურნალობის პერიოდი. პაციენტის ინდივიდუალური საჭიროების მიხედვით შეიძლება დოზის შემცირება 5 მგ-მდე ან გაზრდა მაქსიმალურ რეკომენდებულ დოზამდე-20 მგ. 

სოციალური შფოთვითი აშლილობა არის ქრონიკული დაავადება და რეკომენდებულია 12 კვირიანი მკურნალობის შემდეგ შეფასდეს პაციენტის პასუხი. რეციდივების საპროფილაქტიკოდ 6 ან მეტი თვის განმავლობაში უნდა იქნას გამოყენებული. მკურნალობის ეფექტურობა უნდა გადაისინჯოს რეგულარული დროის ინტერვალებით.

სოციალური შფოთვითი აშლილობა არის სპეციფიური დარღვევის კარგად განსაზღვრული დიაგნოსტიკური ტერმინოლოგია და არ უნდა ავურიოთ ჰიპერთიმიაში. პრეპარატით მკურნალობა ინიშნება მხოლოდ მაშინ თუ დაავადება მნიშვნელოვან ზეგავლენას ახდენს პროფესიულ და სოციალურ მოღვაწეობაზე. 

გენერალიზებული შფოთვითი აშლილობა: ჩვეულებრივ ინიშნება 10 მგ (20 წვეთი) დღეში. პაციენტის ინდივიდუალური საჭიროების მიხედვით შეიძლება დოზის გაზრდა მაქსიმუმადე-20 მგ (40 წვეთი). გენერალიზებული შფოთვითი აშლილობა არის ქრონიკულად მიმდინარე დაავადება და რეკომენდებულია ხანგრძლივი მკურნალობა რეციდივის პრევენციის მიზნით.

ობსესურ-კომპულსური დარღვევა: საწყისი დოზაა 10მგ დღეში ერთხელ.  პაციენტის ინდივიდუალური პასუხის მიხედვით დოზა შესაძლოა გაიზარდოს მაქსიმუმ 20მგ-მდე დღეში. ვინაიდან  მოცემული დაავადება ქრონიკულია,  პაციენტების მკურნლობა უნდა მოხდეს სათანადო პერიოდის განმავლობაში, ვიდრე არ მოხდება სიმპტომების გაქრობა. მკურნალობის წარმატება და დოზა უნდა შეფასდეს რეგულარულად.

ხანდაზმულ პაციენტებში (65 წლის ასაკის ზევით): რეკომენდებული ჩვეულებრივი დოზის ნახევარის გამოყენება (ანუ სულ 5 მგ დღეში) და უფრო დაბალი მაქსიმალური დოზის გამოყენება (10 მგ დღეში).

 

თირკმლის უკმარისობა: დოზის რეგულირება არ არის აუცილებელი მსუბუქი და საშუალო სიმძიმის თირკმლის უკმარისობის დროს. არ არსებობს ინფორმაცია თირკმლის ფუნქციის მძიმე უკმარისობის დროს. (კრეატინინის კლირენსი < 30მლ/წუთში).

ღვიძლის ფუნქციის დარღვევა:

მკურნალობის პირველი ორი კვირის განმავლობაში რეკომენდებულია 5 მგ საწყისი დოზა. შემდეგ დღიური დოზა შეიძლება გაიზარდოს 10 მგ-მდე პაციენტის ინდივიდუალური საჭიროების მიხედვით.

პაციენტები CYP2C19 –ს არასაკამარისი მეტაბოლიზმით: იმ პაციენტებში, რომლებსაც აქვთ CYP2C19 –ს დაქვეითებული მეტაბოლიზმი, მკურნალობის პირველი ორი კვირის განმავლობაში რეკომენდებულია 5 მგ საწყისი დოზა. შემდეგ დღიური დოზა შეიძლება გაიზარდოს 10 მგ-მდე  პაციენტის ინდივიდუალური საჭიროების მიხედვით.

 

მკურნალობის შეწყვეტა: აბსტინენციის შესაძლო რეაქციების პრევენციის მიზნით მკურნალობის დასრულებისას პრეპარატის დოზის კლება უნდა მოხდეს თანდათანობით 1-2 კვირის განმავლობაში.

 

უკუჩვენება:

• მომატებული მგრძნობელობა ესციტალოპრამის ან სხვა ნებისმიერი ინგრედიენტის მიმართ. 

• 18 წლამდე ასაკი

• პრეპარატის გამოყენება უკუნაჩვენებია მონოამინოოქსიდაზას (მაო) ინჰიბიტორებთან ერთად 

 

გვერდითი მოვლენები:

არასასურველი გვერდითი ეფექტები ესპას გამოყენების დროს მსუბუქია და დროებით ხასიათს ატარებს. ისინი გვხვდება მკურნალობის დაწყებიდან პირველი 1-2 კვირის განმავლობაში და ჩვეულებრივ მცირდება დეპრესიული მდგომარეობის გაუმჯობესებისთანავე.

 

განსაკუთრებული  მითითებები:

ესციტალოპრამი შეიძლება დაინიშნოს მაო-ს ინჰიბიტორებით მკურნალობის შეწყვეტის შემდეგ 14 დღეში და A-ტიპის მაოს- ინჰიბიტორის მოკლობემიდის თერაპიის შეწყვეტის შემდეგ მინიმუმ 1 დღეში. უნდა გავიდეს მინიმუმ 7 დღე ესციტალოპრამით მკურნალობის შეწყვეტის შემდეგ და მხოლოდ ამის შემდეგაა შესაძლებელი არასელექტიური მაო-ს ინჰიბიტორებით მკურნალობის დაწყება.

 

ორსულობა და ლაქტაცია:

ესციტალოპრამის უსაფრთხოება ორსულობისა და ლაქტაციის პერიოდში უცნობია. პრეპარატი დედის რძეში ძალიან დაბალი კონცენტრაციით აღწევს. ამ მიზეზით პრეპარატის გამოყენება ორსულობისა და ლაქტაციის პერიოდში არ არის რეკომენდებული, თუ კლინიკური სარგებლიანობა არ აღემატება პოტენციურ რისკს.

 

გულყრები:  პრეპარატის მიღება უნდა შეწყდეს ყველა პაციენტში, რომელსაც განუვითარდა კრუნჩხვა. SSRI- ები არ გამოიყენება არასტაბილური ეპილეფსიის მქონე პაციენტებში, ხოლო კონტროლირებულ ეპილეფსიურ პაციენტებში მხოლოდ საგულდაგულო მონიტორინგის ფონზე შეიძლება SSRI -ის მიღება. 

დიაბეტი: სეროტონონის უკუმიტაცების სელექტიური ინჰიბიტორებით მკურნალობამ შეიძლება შეცვალოს გლიკემიური კონტროლი დიაბეტით დაავადებულ პაციენტებში და საჭირო გახდეს ინსულინის ან/და ორალური ჰიპოგლიკემიური პრეპარატის დოზის კორექცია.
