აღწერა:
ფლუკონაზოლი ქიმიურად წარმოადგენს 2-(2,4 დიფტოროფენი)-1,3 – ბის (1H-1,2,4-ტრიაზოლ-1-ილ) პროპან-2-ოლ. მოლეკულური ფორმულა არის C13H12F2N6O6, ხოლო მოლეკულური მასა – 306.3.

ფარმაკოლოგიური მოქმედება:
ფლუკონაზოლი, როგორც ტრიაზოლის საშუალებების ახალი კლასის წევრი, არის სოკოვანი სტეროლის სინთეზის ძლიერი და სპეციფიკური ინჰიბიტორი. ფლუკონაზოლის აქტივობა გამოვლინდა ოპორტუნისტული მიკოზების წინააღმდეგ, როგორიცაა ინფექციები Candina Spp-ით, მათ შორის ინტრაკრანიალური ინფექციები, აგრეთვე ინფექციები Microsprum Spp-ით და Trichiphyton Spp-ით. 
ფლუკონაზოლი აგრეთვე აქტიური აღმოჩნდა ცხოველთა მოდელებში ენდემური მიკოზებისას, მათ შორის Blastomyces dermatidis, ciccodiodies immitis-ით ინფექციებისას, ინტრაკრანიალური ინფექციების ჩათვლით და ჰისტოპლაზმა Capsulatum-ით ინფექციებისას ნორმალურ და იმუნოსუპრესიულ ცხოველებში.
ფლუკონაზოლი მაღალსპეციფიკურია სოკოვანი P-450 ციტოქრომდამოკიდებული ფერმენტებისათვის. 
დღეში 50 მგ. ფლუკონაზოლის მიღება 28-მდე დღის განმავლობაში გავლენას არ ახდენს მამაკაცებში ტესტოსტერონის პლაზმურ კონცენტრაციებზე და რეპროდუქციული ასაკის ქალებში სტეროიდის კონცენტრაციაზე.
დღეში 200-400 მგ. ფლუკონაზოლს არ გააჩნია კლინიკურად მნიშვნელოვანი მოქმედება ენდოგენურ სტეროიდების დონეებზე ან ACTH-სტიმულირებულ რეაქციაზე ჯანმრთელ მამაკაცებში. პერორალურად მიღების შემდეგ ფლუკონაზოლი კარგად აბსორბირდება და პლაზმური დონეები (და სისტემური ბიოშეღწევადობა) ვენებში შეყვანის შემდეგ არის მიღწეული დონეების 90%. პერორალურ აბსორბციაზე გავლენას არ ახდენს იმავდროულად საკვების მიღება. პლაზმური კონცენტრაციები დოზის პროპორციულია. დოზის დღეში ერთხელ მიღებისას მყარი დონეების 90% მიიღწევა მე-4-5 დღეს.
ჩვეულებრივ დღიურ დოზაზე ორჯერ მაღალი დამტვირთველი დოზის მიღებისას (1-ელ დღეს) პლაზმური დონეები შეადგენს დაახლოებით 90% მყარ დონეებს მე-2 დღისათვის. განაწილების აშკარა მოცულობა უახლოვდება ორგანიზმში არსებული საერთო წყლის მოცულობას. პლაზმურ პროტეინებთან შეკავშირება არის დაბალი (11.12%).
ფლუკონაზოლი კარგად აღწევს ორგანიზმის ყველა შესწავლილ სითხეში. ფლუკონაზოლის დონეები ნერწყვსა და ნახველში პლაზმაში არსებული დონეების ანალოგიურია. ექსკრეციის ძირითადი გზა არის თირკმლისმიერი მიღებული დოზის დაახლოებით 80%-ით, რომელიც უცვლელი პრეპარატის სახით ვლინდება შარდში.
ფლუკონაზოლის კლირენსი კრეატინინის კლირენსის პროპორციულია. მოცირკულარე მეტაბოლიტების არსებობის ფაქტები არ არსებობს.
ხანგრძლივი პლაზმური ნახევარგამოყოფის პერიოდი უზრუნველყოფს ერთჯერადი დოზის გამოყენებას ვაგინალური კანდიდოზის სამკურნალოდ, ხოლო სხვა ჩვენებებისას დოზის დღეში ერთხელ და კვირაში ერთხელ მიღებას.

ჩვენებები:
ფლუკონაზოლით მკურნალობა შეიძლება დაიწყოს კულტურებისა და სხვა ლაბორატორიული გამოკვლევების შედეგების მიღებამდე, თუმცა, ამ შედეგების მიღების შემდეგ აუცილებლად უნდა მოხდეს ანტიინფექციური თერაპიის დაწყება.

კრიპტოკოკოზი:
მათ შორის კრიპტოკოკური მენინგიტი და ინფექციები სხვა ადგილებში (მაგ. პულმონური, კანის). შეიძლება იმ პაციენტთა მკურნალობა, რომლებსაც აქვთ შიდსი, ტრანსპლანტირებული ორგანო ან იმუნოსუპრესიის სხვა გამომწვევი მდგომარეობები. ფლუკონაზოლი შეიძლება გამოყენებული იყოს შემანარჩუნებელი თერაპიის სახით შიდსის მქონე პაციენტებში კრიპტოკოკური დაავადების რეციდივის პრევენციისათვის.

სისტემური კანდიდოზი:
მათ შორის კანდიდემია, დისემინირებული კანდიდოზი და ინვაზიური კანდიდოზური ინფექციების სხვა ფორმები, რომლებიც მოიცავს მუცლის, ენდოკარდიუმის, თვალის, ფილტვისა და საშარდე ტრაქტის ინფექციებს. შეიძლება კანდიდოზური ინფექციებისადმი წინასწარგანწყობილი პაციენტების მკურნალობა. 

მუკოზური კანდიდოზი:
მოიცავს ოროფარინგულ, ეზოფაგურ, არაინვაზიურ ბრონქოპულმონურ ინფექციებს, კანდიდურიას, მუკოკუტანურ და ქრონიკულ ორალურ ატროფიულ კანდიდოზს. შეიძლება კომპრომისული იმუნური ფუნქციის მქონე პაციენტთა მკურნალობა. გამოიყენება შიდსის მქონე პაციენტებში ოროფარინგული კანდიდოზის რეციდივის პრევენციისათვის.

სასქესო ორგანოების კანდიდოზი:
ვაგინალური კანდიდოზი, მწვავე ან მორეციდივე და პრევენცია ვაგინალური კანდიდოზის რეციდივის შესამცირებლად (წელიწადში 3 ან მეტი ეპიზოდი), კანდიდოზური ბალანიტი. 

სოკოვანი ინფექციის პრევენცია:
ავთვისებიანობის მქონე პაციენტებში, რომლებიც წინასწარგანწყობილნი არიან ამგვარი ინფექციებისადმი ციტოტოქსიკური ქიმიოთერაპიის ან რადიოთერაპიის შედეგად.

დერმატომიკოზი:
მათ შორის ტინეა პოდეს, ტინეა ცორპორის, ტინეა ვერსიცოლორ, ტინეა უნგუიუმ (ონიქომიკოზი) და კანის კანდიდა ინფექციები.

ფარმაკოკინეტიკა:
კარგად აბსორბირდება პერორალური მიღების შემდეგ. ფუნგიციდური კონცენტრაციები მიიღწევა ფრჩხილებში, ნერწყვსა და ვაგინაში. აღწევს ასევე ტვინში.
მოქმედების დაწყება: 
მრავალჯერადი დოზების მიღების შემდეგ მყარი კონცენტრაციები მიიღწევა 6-10 დღეში.
მოქმედების ხანგრძლივობა: 24 საათამდე.
უკუჩვენება: ჰიპერმგრძნობელობა ფლუკონაზოლისადმი.

სპეციალური სიფრთხილის ზომები:
ღვიძლის დისფუნქცია, 13 წლამდე ასაკის ბავშვები.
ორსულობა: გამოიყენება მხოლოდ იმ შემთხვევაში, თუ პოტენციური სარგებელი უფრო მნიშვნელოვანია, ვიდრე შესაძლო რისკი ნაყოფისათვის.
ლაქტაციის პერიოდი: რეკომენდებული არ არის.
ხანდაზმული პაციენტები: გამოიყენება სიფრთხილით.

ურთიერთქმედებები: 
ციმეტიდინი: ფლუკონაზოლის ეფექტიანობის დაქვეითება.
ციკლოსპორინი: მატულობს შრატისმიერი ციკლოსპორინის პლაზმური კონცენტრაცია.
ჰიდროქლორთიაზიდი: ქვეითდება ფლუკონაზოლის თირკმლისმიერი კლირენსი.
ფენიტოინი: ძლიერდება ფენიტოინის ეფექტიანობა.
რიფამპიცინი: ქვეითდება ფლუკონაზოლის ეფექტიანობა, საჭიროა უფრო მაღალი დოზირება.
სულფონილშარდოვანა: იზრდება ტოლბუტამიდის, გლიბურიდისა და გლიპიზიდის ეფექტიანობა. 
ვარფარინი: აძლიერებს ანტიკოაგულანტურ მოქმედებას, რაც იწვევს პროთრომბინის დროის გაზრდას.

დოზირება:
მოზრდილები:
მუკოზური: 50-100 მგ. დღეში 14-30 დღის განმავლობაში.
ვაგინალური: 150 მგ. ერთჯერადი პერორალური დოზის სახით.
სისტემური: 400 მგ. პირველ დღეს დამტვირთველი დოზის სახით, 200-400 მგ. დღეში ერთხელ 10-12 კვირის განმავლობაში CSF-ის სტერილურობის შემდეგ. სოკოვანი ინფექციების პროფილაქტიკა (იმუნოკომპრომისულ/რისკის ქვეშ მყოფ პაციენტებში): 50-100 მგ დღეში ერთხელ. დოზების მიღება შეიძლება პერორალურად ან ი/ვ ინფუზიით სიჩქარით 5-10 მლ/წთ.
ბავშვები: სერიოზული სიცოცხლისათვის საშიში ინფექციები 1 წლის ზემოთ: 3-6 მგ/კგ სხეულის წონაზე დღეში.

პრეპარატები უნდა ინახებოდეს ბავშვებისათვის მიუწვდომელ ადგილას.
