**მოქმედების მექანიზმი**

მიღების წესები და დოზები

განსაკუთრებული მითითებები
პრეპარატ ამრა ფორტეს მიღებამ პირველი 2 კვირის განმავლობაში, შესაძლებელია არტერიული წნევის მკვეთრი დაქვეითება გამოიწვიოს, განსაკუთრებით მისი პირველი მიღებისას. არტერიული წნევის მკვეთრი დაქვეითების რისკი იზრდება პაციენტებში საერთო მოცირკულირე სისხლის მოცულობის (სმსმ) შემცირებით, გულის მძიმე უკმარისობით, დაბალი საწყისი არტერიული წნევით, თირკმლის არტერიების სტენოზით ან ერთი მოფუნქციონირე თირკმლის არტერიის სტენოზით, ღვიძლის ციროზით (შეშუპებითა და ასციტით).
პირველი მიღებისას განვითარებული არტერიული წნევის მკვეთრი დაქვეითება არ წარმოადგენს პრეპარატით მკურნალობის შეწყვეტის მიზეზს. სმსმ-ისა და ა/წ-ის აღდგენისას შესაძლებელია პრეპარატის ნაკლები დოზით ან შემადგენელი კომპონენტების მონოთერაპიით გაგრძელება.
ამრა ფორტეს გამოყენებისას საჭიროა ელექტროლიტებისა და კრეატინინის კონცენტრაციის სისტემატური კონტროლი. უსაფრთხოების ზომები პერინდოპრილის გამოყენებისას ამფ-ის ინჰიბიტორების, მათ შორის პერინდოპრილის მიღებისას ზოგჯერ შესაძლებელია სახის ქვედა ნაწილის, ტუჩების, ენის, ხახისა და ხორხის ანგიონევროზული შეშუპების განვითარება. აღნიშნული სიმპტომების გამოვლინებისას საჭიროა პერინდოპრილის მიღების დაუყოვნებლივ შეწყვეტა და ექიმის კონსულტაცია. ამფ-ის ინჰიბიტორებით მკურნალობისას არ შეიძლება ჰემოდიალიზის ჩატარება პოლიაკრილის მემბრანების გამოყენებით. პაციენტებში გულის იშემიური დაავადებით და ტვინის სისხლმომარაგების უკმარისობით მკურნალობა იწყება პრეპარატის მცირე დოზებით.
პაციენტებში გულის გამოხატული უკმარისობით (IV სტადია) და ინსულინდამოუკიდებელი შაქრიანი დიაბეტით (კალიუმის დონის სპონტანური მატების საშიშროება) მკურნალობა იწყება მცირე დოზებით და ექიმის მუდმივი მეთვალყურეობის ქვეშ. ზოგადი ანესთეზიის დროს ამფ-ის ინჰიბიტორების გამოყენებამ შესაძლოა არტერიული წნევის დაქვეითება გამოიწვიოს, განსაკუთრებით მაშინ, როცა ანესთეზიურ საშუალებას ჰიპოტენზიური მოქმედება ახასიათებს. ამ შემთხვევაში ქირურგიულ ჩარევამდე 12 საათით ადრე საჭიროა პერინდოპრილის მოხსნა.
**უსაფრთხოების ზომები ინდაპამიდის გამოყენებისას**
პაციენტებში ღვიძლის ფუნქციების დარღვევით ინდაპამიდის გამოყენებამ შესაძლოა ღვიძლის ენცეფალოპათია გამოიწვიოს. ამ შემთხვევაში პრეპარატის მიღება დაუყოვნებლივ წყდება. მკურნალობის დაწყებამდე და მკურნალობის დროს საჭიროა სისხლის პლაზმაში ნატრიუმის კონცენტრაციის კონტროლი. ინდაპამიდს, როგორც თიაზიდურ დიურეზულ საშუალებას, ახასიათებს შარდთან ერთად კალციუმის გამოდევნის შეზღუდვა, რაც სისხლში კალციუმის კონცენტრაციის დროებით მატებას იწვევს. შაქრიანი დიაბეტით დაავადებულებში აუცილებელია სისხლში გლუკოზის დონის მუდმივი კონტროლი, განსაკუთრებით ჰიპოკალიემიის ფონზე. პაციენტებში სისხლის პლაზმაში შარდმჟავას მაღალი შემცველობით ინდაპამიდი ზრდის ნიკრისის ქარის (პოდაგრას) განვითარებას. პრეპარატის ეფექტურობა და უსაფრთხოება ბავშვებში დადგენილი არ არის. არ არის რეკომენდებული პრეპარატის გამოყენება ბავშვებსა და 18 წლამდე მოზარდებში.
დოზის გადაჭარბების სიმპტომები: ა/წ გამოხატული დაქვეითება, გულისრევა, ღებინება, თავბრუსხვევა, პოლიურია ან ოლიგურია (ჰიპოვოლემიის შედეგად ანურიაში გარდამავალი), ბრადიკარდია, ელექტროლიტური დისბალანსი. მკურნალობა: კუჭის ამორეცხვა, ადსორბენტების დანიშვნა, წყალ-მარილოვანი ბალანსის კორექცია. ამრა ფორტე არ გამოიყენება ლითიუმთან, კალიუმშემნახველ დიურეზულ საშუალებებთან, კალიუმის პრეპარატებთან, საანესთეზიო საშუალებებთან, ალოპურინოლთან, ციტოსტატიკებთან, იმუნოდეპრესიულ და სისტემურ კორტიკოსტეროიდულ საშუალებებთან კომბინაციაში.
ერითრომიცინთან (ი/ვ გამოყენებისათვის), პენტამიდინთან, სულტოპრიდთან, ვინკამინთან, ბეპრიდილთან ერთად გამოყენებისას შესაძლებელია “პირუეტის” ტიპის არითმიის განვითარება. ანთების საწინააღმდეგო არასტეროიდული საშუალებები (ასას) ამცირებენ ამფ-ის ინჰიბიტორების მოქმედებას. ასას და ამფ-ის ინჰიბიტორებს ახასიათებთ ჰიპერკალიემიის განვითარებისადმი ადიტიური მოქმედება, ამასთანავე შესაძლებელია თირკმლის ფუნქციის დაქვეითება გამოიწვიონ.
ამრა ფორტესა და ტრიციკლური ანტიდეპრესანტების, ნეიროლეფსიური საშუალებების ერთდროული გამოყენებისას იზრდება ჰიპოტენზიური ეფექტი და ორთოსტატური ჰიპოტენზიის განვითარების რისკი (ადიტიური ეფექტი). ინდაპამიდის I-ა კლასისი (ქინიდინი, ჰიდროქინიდინი, დიზოპირამიდი) და III კლასის (ამიოდარონი, სოტალოლი) ანტიარითმიულ პრეპარატებთან გამოყენებამ შესაძლოა “პირუეტის” ტიპის არითმია გამოიწვიოს. საფაღარათო საშუალებების დანიშვნის აუცილებლობისას ნაწლავის მოტორიკის გამაძლიერებელი საშუალებები არ გამოიყენება. ამრა ფორტეს საგულე გლიკოზიდებთან კომბინაციაში დანიშვნისას საჭიროა კალიუმის დონის და ეკგ-ის კონტროლი და აუცილებლობისას - თერაპიის კორექტირება.
შენახვის პირობები და ვადები
