შემადგენლობა
სტაზექსის ერთი ტაბლეტი შეიცავს აქტიური ნივთიერების - კლოპიდოგრელის 75 მგ-ს.
**მოქმედების მექანიზმი**
თრომბოციტების აგრეგაციის ინჰიბიტორი. კლოპიდოგრელი ამორჩევითად აბლოკირებს ადენოზინ-დიფოსფატის (ადფ) მიერთებას თრომბოციტების შესაბამის რეცეპტორებთან, ასევე - ადფ-ის მოქმედებით გამოწვეული GPIIბ/IIა კომპლექსის აქტივაციას. კლოპიდოგრელი ასევე აინჰიბირებს სხვა აგონისტებით გამოწვეული თრომბოციტების აგრეგაციას, რაც ადფ-ით გამოთავისუფლებული თრომბოციტების აქტივობის ბლოკადით მიიღწევა. კლოპიდოგრელი შეუქცევადად უერთდება თრომბოციტების ადფ-რეცეპტორებს და შესაბამისად თრომბოციტები არააქტიურნი ხდებიან ადფ-ის მიმართ მთელი სასიცოცხლო ციკლის განმავლობაში. თრომბოციტების ნორმალური ფუნქციის აღდგენის სისწრაფე შეესაბამება მათი განახლების სისწრაფეს. თრომბოციტების აგრეგაციის მნიშვნელოვნად დათრგუნვის ეფექტი ვლინდება პრეპარატის მიღების პირველივე დღიდან, იგი ძლიერდება და სტაბილურ მდგომარეობას აღწევს 3-7 დღის შემდეგ. ყოველდღიურად კლოპიდოგრელის 75 მგ მიღებისას, თრომბოციტების აგრეგაციის ინჰიბირების საშუალო მაჩვენებელი 40-60%-ია. თრომბოციტების აგრეგაცია და სისხლდენის დრო საწყის მაჩვენებლებს უბრუნდება პრეპარატით მკურნალობის შეწყვეტიდან საშუალოდ 5 დღის შემდეგ. კლოპიდოგრელს ახასიათებს კორონაროგამაფართოებელი მოქმედება. სისხლძარღვთა ათეროსკლეროზული დაზიანების დროს, პროცესის ლოკალიზაციის მიუხედავად, (ცერებროვასკულური, კარდიოვასკულური თუ პერიფერიული დაზიანება) ხელს უშლის ათეროთრომბოზის განვითარებას. კლოპიდოგრელი პერორალური მიღების შემდეგ სწრაფად აბსორბირდება კუჭ-ნაწლავის ტრაქტიდან. ღვიძლში განიცდის ბიოტრანსფორმაციას. ძირითადი მეტაბოლიტი, რომელიც პლაზმაში მოცირკულირე შენაერთების 85%-ს შეადგენს, არააქტიურია. აქტიური მეტაბოლიტი კლოპიდოგრელის 2-ოქსი-კლოპიდოგრელამდე დაჟანგვით და მისი შემდგომი ჰიდროლიზით მიიღება. აქტიური მეტაბოლიტი პლაზმაში არ აღინიშნება.
ჩვენებები
იშემიური დარღვევების პროფილაქტიკა პაციენტებში ათეროსკლეროზული ცვლილებით, მათ შორის:
მიოკარდიუმის ინფარქტის გადატანის შემდგომ პერიოდში, იშემიური ინსულტის ან პერიფერიული არტერიების დაავადებების დროს;
- მწვავე კორონარული სინდრომით შეპყრობილ პაციენტებში შთ-სეგმენტის სიმაღლის მომატების გარეშე (არასტაბილური სტენოკარდია ან მიოკარდიუმის ინფარქტი პათოლოგიური Q-კბილის გარეშე) აცეტილსალიცილის მჟავასთან კომბინაციაში.
მიღების წესები და დოზები
მოზრდილებსა და ხანდაზმულ პაციენტებში ინიშნება 75 მგ(1 ტაბლეტი) დღეში ერთხელ, საკვების მიღებისგან დამოუკიდებლად. მწვავე კორონარული სინდრომით შეპყრობილ პაციენტებში შთ-სეგმენტის სიმაღლის მომატების გარეშე (არასტაბილური სტენოკარდია ან მიოკარდიუმის ინფარქტი პათოლოგიური Q-კბილის გარეშე) მკურნალობა იწყება ერთჯერადი დოზით 300 მგ(4 ტაბლეტი), შემდეგ გრძელდება 75 მგ(1 ტაბლეტი) დღეში ერთხელ (აცეტილსალიცილის მჟავასთან ერთად). ვინაიდან აცეტილსალიცილის მჟავას დიდი დოზით გამოყენება სისხლდენის განვითარების მნიშვნელოვან რისკ-ფაქტორს წარმოადგენს, აცეტილსალიცილის მჟავას რეკომენდებულმა დოზამ არ უნდა გადააჭარბოს 100 მგ-ს. პრეპარატით მკურნალობა ინფარქტის გადატანის შემდეგ იწყება რამდენიმე დღიდან - 35 დღის პერიოდში, ხოლო იშემიური ინსულტის შემთხვევაში - 7 დღიდან 6 თვემდე პერიოდში.
გვერდითი მოვლენები
სისხლის შემდედებელი სისტემის მხრივ: სისხლჩაქცევები კუჭ-ნაწლავის ტრაქტში, რომელიც ძალიან იშვიათად მოითხოვს პაციენტის ჰოსპიტალიზაციას, უფრო ხშირად _ ჰემატომები, ჰემატურია ან სისხლჩაქცევა კონიუნქტივაში; სისხლმბადი სისტემის მხრივ: ნეიტროპენია, თრომბოციტოპენია; საჭმლის მომნელებელი სისტემის მხრივ: აბდომინალური ტკივილი, დისპეფსია, გასტრიტი, ყაბზობა, ფაღარათი ან გულისრევა; დერმატოლოგიური რეაქციები: კანზე გამონაყარი, ქავილი; ცენტრალური და პერიფერიული ნერვული სისტემის მხრივ: თავის ტკივილი, თავბრუსხვევა, პარესთეზიები; ღვიძლისა და სანაღვლე სისტემის მხრივ: იშვიათად ღვიძლის ფუნქციური სინჯების ნორმიდან გადახრა; ძალიან იშვიათად ვლინდება: ბრონქოსპაზმი, ანგიონევროზული შეშუპება, ანაფილაქსიური რეაქცია.
უკუჩვენება
- ღვიძლის ფუნქციის მძიმე დარღვევები;
- მწვავე სისხლდენა, გამოწვეული პეპტიური წყლულით ან თავის ტვინში სისხლჩაქცევით;
- ჰიპერფიბრინოლიზი;
- კუჭისა და თორმეტგოჯა ნაწლავის წყლულოვანი დაავადება გამწვავების ფაზაში;
- არასპეციფიკური წყლულოვანი კოლიტი;
- ტუბერკულოზი;
- ფილტვის კიბო;
- ორსულობისა და ლაქტაციის პერიოდი;
- 18 წლამდე ასაკის პაციენტები (არ არის დადგენილი უსაფრთხოება და ეფექტურობა);
- პრეპარატის შემადგენელი კომპონენტებისადმი მაღალი მგრძნობელობა;
პრეპარატი სიფრთხილით ინიშნება ღვიძლისა და თირკმლების დაავადებების დროს (ღვიძლის ან თირკმლის საშუალო სიმძიმის უკმარისობა), ტრავმებისა და ოპერაციის წინა პერიოდში.
განსაკუთრებული მითითებები
სისხლდენის განვითარებისა და ჰემატოლოგიური ხასიათის გვერდითი მოვლენების რისკის არსებობის გამო, კლინიკური სიმპტომების გამოვლინებისთანავე საჭიროა სისხლისა (თრომბოციტების რაოდენობა, თრომბოციტების ფუნქციური აქტივობის ტესტები და სხვა) და ღვიძლის ფუნქციური აქტივობის ანალიზების ჩატარება. ქირურგიული ჩარევების წინ კლოპიდოგრელი უნდა მოიხსნას ოპერაციამდე 7 დღით ადრე. პრეპარატი სიფრთხილით გამოიყენება პაციენტებში სისხლდენის (განსაკუთრებით კუჭ-ნაწლავის ტრაქტში და თვალშიდა სისხლდენა) რისკით. პაციენტები ინფორმირებულნი უნდა იყვნენ, კლოპიდოგრელით მკურნალობის ფონზე სისხლდენის ნებისმიერი გამოვლინებისთანავე მიმართონ ექიმს. მიოკარდიუმის მწვავე ინფარქტისა (პათოლოგიური Q-კბილით) და იშემიური ინსულტის დროს კლოპიდოგრელით მკურნალობის დაწყება დაავადების მწვავე პერიოდში არ არის რეკომენდებული. აცეტილსალიცილის მჟავა არ მოქმედებს კლოპიდოგრელის ეფექტზე, ხოლო კლოპიდოგრელი ზრდის აცეტილსალიცილის მჟავას მოქმედებას თრომბოციტების კოლაგენ-ინდუცირებულ აგრეგაციაზე. კლოპიდოგრელისა და აცეტილსალიცილის მჟავას ერთდროულად ხანგრძლივი პერიოდით გამოყენების უსაფრთხოება დადგენილი არ არის. კლოპიდოგრელი არ ცვლის ჰეპარინის მოთხოვნილებასა და სისხლის შემდედებელ სისტემაზე მის მოქმედებას. ასევე არ ახდენს გავლენას ჰეპარინი კლოპიდოგრელის მაინჰიბირებელ ეფექტზე. თუმცა, ამ ორი პრეპარატის ერთდროულად გამოყენების უსაფრთხოება შესწავლილი არ არის და მათი კომბინაცია მოითხოვს სიფრთხილეს. კლოპიდოგრელისა და ფიბრინოლიზური საშუალებების ერთდროულად გამოყენების უსაფრთხოება ბოლომდე დადგენილი არ არის, ამიტომ მათი კომბინაცია მოითხოვს სიფრთხილეს. კლოპიდოგრელისა და ვარფარინის ერთდროული გამოყენება მოსალოდნელი სისხლდენის განვითარების გამო არ არის რეკომენდებული. ანთების საწინააღმდეგო არასტეროიდული საშუალებები(ასას) კლოპიდოგრელთან კომბინაციაში მოითხოვს სიფრთხილეს. კლოპიდოგრელი კლინიკური მნიშვნელობის გავლენას არ ახდენს ატენოლოლზე, ნიფედიპინზე, ფენობარბიტალზე, ციმეტიდინზე, ესტროგენებზე, დიგოქსინზე, თეოფილინზე, ფენიტოინზე, ტოლბუტამიდსა და ანტაციდურ საშუალებებზე. გლიკოპროტეინ IIბ/IIIა ინჰიბიტორებთან ერთად კლოპიდოგრელი სიფრთხილით ინიშნება. კლოპიდოგრელით მკურნალობის პირველ კვირაში სისხლის ანალიზი ტარდება კლოპიდოგრელისა და აცეტილსალიცილის მჟავას, ასას-ს, ჰეპარინის, გლიკოპროტეინ IIბ/IIIა ინჰიბოტორების ან ფიბრინოლიზური საშუალებების კომბინაციის შემთხვევაში. ასევე - პაციენტებში სისხლდენის რისკით, დაკავშირებული ტრავმებთან, ქირურგიულ ჩარევებსა და სხვა პათოლოგიურ მდგომარეობებთან. პრეპარატი არ მოქმედებს სატრანსპორტო საშუალებების მართვის უნარსა და ყურადღების კონცენტრაციაზე.
ჭარბი დოზირება
პრეპარატის მაღალი დოზით გამოყენებისას არ არის დადგენილი სერიოზული გვერდითი მოვლენები. გახანგრძლივებული სისხლდენის სწრაფი კორექციის მიზნით, რეკომენდებულია თრომბოციტული მასის გადასხმა. სპეციფიკური ანტიდოტი არ არსებობს.
შენახვის პირობები და ვადები
პრეპარატის ვარგისიანობის ვადაა 3 წელი, ინახება 10-25oC ტემპერატურაზე, სინათლისაგან დაცულ, მშრალ, ბავშვებისათვის ხელმიუწვდომელ ადგილზე!
